About the ward

The Cardiac Unit specialises in the acute care of patients with cardiac conditions. The unit contains 27 beds incorporating seven coronary care beds. Facilities on the unit include a range of invasive and non-invasive monitoring with an exercise testing room. The unit is also home to the UK’s first integrated Heart Attack and Stroke Thrombolysis Service.

We have innovated the role of The Heart Attack and Stroke or HAS nurse who is a cardiac trained nurse is based on the CCU who responds to the switchboard callout when a patient presents through our direct access pathway for Heart Attack and Stroke. The patient suffering a heart attack will receive primary PCI and be transferred directly to CCU. Similarly the patient with an acute stroke receives thrombolysis and is transferred directly to CCU. A bed is kept available at all times for these patients as the service runs for 24 /7.

The HAS nurse is a trained cardiac nurse with several years of cardiac nursing experience and the skills to provide advanced life support as necessary. This includes recognition of life-threatening cardiac rhythms on the monitor, defibrillation and the administration of first line emergency drugs.

We carry out a range of investigations and treatments including cardiac catheterisation, coronary angioplasty, insertion of coronary stents, insertion of temporary and permanent pacing systems, cardioversion, intra-aortic balloon pumping, invasive monitoring via central and arterial catheters, and non-invasive ventilatory support. Many of our patients are classified as Level 2 critical care requiring high dependency nursing.

The inpatient cardiac ward adjacent to the CCU forms part of the national pulmonary hypertension centre at the Royal Free and we receive patients who have acutely decompensated, for diagnosis, treatment and long-term management of this complex disease. The unit is the designated national centre for pulmonary hypertension associated with connective tissue disease.

We run a weekly pre-admission clinic on the ward for patients undergoing PCI, Angiography or permanent pacemaker insertion/box change and Internal Cardioverter Defibrillator (ICD) insertion on an elective basis.

The ward team is lead by a Senior Sister, Junior Sister/ Charge Nurses and an experienced team of qualified cardiac nurses, supported by the Matron for Acute Medicine. The cardiac nursing team also includes a clinical nurse specialist (CNS) and a specialist sister in cardiology who are both responsible for the rapid access chest pain clinic, a CNS for cardiovascular risk management, a team responsible for cardiac rehabilitation and a cardiac clinical audit nurse.

The nursing staff work in teams, which rotate through all areas of the Unit. This provides continuity of care and ensures that nurses have the opportunity to gain experience in all areas of the Unit.

Professional development and support

A mentor supports each member of staff. Performance development planning and a staff development programme are used to guide individual nurse’s progress.

The programme includes:

♥ Preceptorship for newly qualified nurses

♥ A Staff Nurse (Band 5) development package to prepare nurses without cardiac experience for caring for the acutely ill patients

♥ development packages for the Senior Staff Nurse and Junior Sisters grades

♥ A unit based teaching programme that facilitates some of the learning required by the development packages

♥ Support from a Practice Educator

♥ Access to courses at the staff education and development centre within the trust, which provides a variety of clinical and professional education including a diploma in management studies

♥ Support for other higher education activities including cardiac degree places at Kings University.

Practice development is valued highly within the trust and quality care is supported through implementation of Integrated Care Pathways within the Cardiac Unit.

The team also works closely with the North Central London Cardiac Network project leaders, to achieve improved care for cardiac patients and the outcomes required by the national service framework standards.

Full time posts require a 37½ hr week involving internal rotation to night duty working long days.

The Trust is based in Hampstead, which provides a variety of bars and restaurants that are used regularly by the team on the cardiac unit!
